You will contribute your Occupational Therapy expertise to the development and delivery of a comprehensive, integrated and multidisciplinary Community Health Services within the Barossa Valley region. Coordinating an increasingly complex case load, and supporting other team members in managing the demands of the service, you will deliver services which promote self-care and personal responsibility, and are appropriate to the local context. This will be achieved by developing and maintaining close working relationships with local case managers, Paramedical Aides, Allied Health Professionals, Nursing and other staff and applying Primary Health Care and Community Participation principles to ensure services are responsive to changing community needs and improve health outcomes. Supporting intake points to identify appropriate candidates for Community Health Services and its programs, you will strive to improve client outcomes for Out of Hospital services and contribute to the review, development and adaptation of clinical and administrative resources to support the Out of Hospital Team.
Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia. In addition, you will have proven experience in supporting clients, carers and families across the Patient Journey, providing effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow-up. You will work well as part of a multidisciplinary team, utilising a combination of preventative, early intervention, treatment/therapy and evaluation approaches. You will be committed to applying client-centred practice and community engagement principles in development and delivery of occupational therapy services, ensuring clients are meaningfully involved in all aspects of their care.
